Activities Passport

SCIP is working in partnership with pinpoint and Living Sport on an Activities Passport. The passport is based on the very popular Hospital Passport – for details about the Health Passport go to: http://www.cuh.org.uk/addenbrookes-hospital/for-patients/patient-information-and-consent-forms/for-people-with-disabilities-and-their-carers

The Activities Passport idea came from parents/carers. They asked for something they could use when their child went to an activity, joined the Scouts/Guides, drama, karate, etc.

We are trying to make the booklet as simple and concise as possible. The Activities Passport will collect information you want/need organisers of activities to know about your child. It will hold details about their likes, dislikes, emergency contact details and lots more.
We are asking families and professionals to take a look at the passport and tell us what you think of it. We want this to work for you.
This is the first draft – at this stage we would like to hear what you like/don’t like, is missing from the passport. All of your thoughts and ideas will help us to develop it into a useful and handy booklet.
Please send your feedback to Joan Adamson: SCIP@cambridgeshire.gov.uk
